Leviton Quickport 12-port open patch block face (this is a discontinued item, but I believe that the original part number was 49Q89-12J). Designed for use with Leviton Quickport jacks and can be mounted with a standard 89D bracket (not included). Great for custom installations where a variety of jacks are required. Bulk packed loose with no individual packing.
