Leviton 41054-IDD ivory modular phone jack insert plate. Type 105, 6-position, 4-conductor flush mount jack platform fits a standard single opening electrical wallplate (1.406" diameter opening). Mounts easily in standard single-, double-, or multi-gang electrical boxes, most floor monuments, and many cellular or raised floor boxes. Installs quickly using industry standard 110-type insulation displacement connectors for 22-26 AWG inside wiring (requires punchdown tool - sold separately).
