Leviton 41090-SMG gray multimedia 4-port wallplate for use with Quickport and multimedia jacks. Angled lower ports to reduce bend radii of inserted patch cords; particularly important for fiber optic applications. Each includes 2 blank inserts and a divider for the lower duplex port to allow installation of two jacks or a duplex MOS plate. Inset areas with clear plastic covers so that you can label each port.
