Leviton C5159 VHF/UHF and FM signal combiner. Used to combine one 75 ohm signal (cable) and one 300 ohm signal (VHF/UHF/FM) into a single 75 ohm output. Female 75 ohm jack on the back is meant to attach directly onto the back of a television or VCR/DVD/cable box. Each is individually packaged with instructions on the back.
