Leviton 5F100-6MC OPT-X fiber adapter plate loaded with 12 fiber optic MT-RJ couplings. These are dual polarity and are MM rated. Designed for installation in Leviton fiber optic patch panels. Also fits the Leviton 47612-MMA multi-media bracket (sold separately) to create custom audio/video panels in a Structured Media Center (SMC).
