Hubbell PT4XCGY2C gray replacement poke-through subplate/cover. I believe that this would be considered a subplate and would nbe installed under a metal protecting cover. Has 4 openings for receptacles and 4 opening for keystone jacks. Includes 4 blank keystone inserts.
